Citizenship Evidence

Citizenship Evidence, opens a new window and photocopy of Citizenship Evidence.

Photo ID

Photo ID, opens a new window and photocopy of Photo ID (front and back). For minor applications, use parent's photo ID

Passport Photo

One (1) 2x2 passport photo. Must meet photo requirements, opens a new window.

Passport photo service is available on-site for $12. Please indicate if you will need a photo taken when making your appointment.

Fees: Two separate Payments

Passport Fee: Check or money order only. Must be made out to "U.S. Department of State". To see how much you will pay, please visit Passport Application Fees.

Acceptance Fee: $35 per application paid to "Commerce Public Library". All forms of payment accepted.
